For routine refills please contact your pharmacy to request a refill electronically. Due to adverse risks of medications, a patient requesting renewals of medication who has not been examined within the preceding 90 days, will not be refilled. Related to certain medications, patients will be required to complete blood test every three months to allow continued usage of some medications and provide renewals.
Co-payment and past balances are due in full at the time of the check-in for all visits. Please confirm your visit with your health insurance before arriving to the visit. You are responsible for any fees including coinsurance and deductible for the services performed if they are not covered by your insurance. A detailed financial consent form will be provided and required to be signed on your first visit.
