What is a DOT Physical

A Department of Transportation (DOT) physical is a health examination mandated by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle (CMV) drivers. A DOT physical helps determine if a driver is physically, mentally, and emotionally fit to operate a commercial vehicle.

East Cary Family Physicians is now offering DOT physical exams needed for your Commercial Driver's License application. NC DOT physical exams for CDL are done by Anna Brey, PA-C (Certified Medical Examiner).

What We Offer

  • Exam Cost: $100 (this is not covered by insurance and will be collected before the appointment)
  • A Department of Transportation (DOT) physical examination would be conducted by Lara Schoenebeck, FNP at East Cary Family Physicians. She is a licensed "medical examiner" listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry
  • Medical Examination Report (MER) form MCSA-5875
  • Revised Medical Examiner’s Certificate (MEC form MCSA-5876)

    After hours, contact the # above for URGENT Needs ONLY and your call would be answered by the on-call doctor's voice mail, who will return your call promptly. Please leave a brief message on the voice-mail so the doctor is able to contact you.

    If you have a non-urgent question, you can send your provider a message. Log into the patient portal, click message and "Ask Doctor". Please give us 24-48 hours to respond.

    UNC Urgent Care at Cary is available after hours to provide treatment for our patients. UNC Urgent Care at Cary has a co-management agreement for our patients after office hours. Our providers have access to your chart, urgent care staff, and may triage if needed. Urgent Care will fax notes to your provider if there is a change in care or medications
